Marionette Lines: Causes, Assessment & Treatment Options
The fold that runs from the corner of the mouth downward is rarely one thing. Volume that has fallen away around the mouth and chin, tissue that has slid down from the cheek, and a muscle that pulls the mouth corner down every time you speak are usually acting together, and each answers to a different treatment. The lower face is also the area where repeated filler accumulates most visibly over years, which is why the plan matters as much as the choice of treatment. This site is about working out what is actually folding, what each option can change, and what none of them can.
Patient education reviewed by Tongchun Mao, MD. A mouth corner that has dropped suddenly, one-sided weakness, numbness or a change in the bite should be assessed medically before anything cosmetic is considered.
Six things that deepen the fold beside the mouth
The first three are almost always present together, and naming which one dominates is what decides the treatment. The last two are the reason an assessment comes before a plan. The commonest mistake in this area is treating the surface of a fold whose cause is somewhere else.
Volume that has fallen away
Fat and bone support are lost around the mouth and the chin as the face changes, so the tissue that used to be full sits against a smaller frame. The fold appears because there is less beneath it, and it is the cause that adding volume is meant for. It is also the cause most often treated when the real problem is descent.
Tissue that has slid down
Cheek and jaw tissue descends, and the skin that was over the cheekbone is now resting beside the mouth. The fold is being folded by tissue arriving from above rather than by anything lost beneath it. This is the cause where adding volume to the fold does least, and where lifting does most.
A muscle that pulls the corner down
A muscle runs from the jaw to the corner of the mouth and pulls it downward, and it does so every time you speak, smile or make an expression. It deepens the fold all day and it is the reason no treatment erases it. Reducing that pull is a genuine option, and it does not replace volume or lift anything.
A chin that does not support the fold
Where the chin recedes, the fold beside it looks deeper than it is, because the eye reads the whole area against the projection of the chin. This is a structural cause rather than a soft-tissue one, and it is why the assessment looks at the profile rather than only at the front of the face.
Skin that folds more easily than it did
Skin loses elasticity and becomes thinner, so the same fold in the same frame reads more deeply than it once did. This is the reason a fold that was barely visible at forty is obvious at sixty. Skin care improves the surface, and no topical changes how the fold is created.
Usually all three at once
Volume loss, descent and muscle pull develop together and reinforce each other, which is why treating one of them frequently produces an improvement that satisfies nobody. Working out which dominates is what allows a plan to be built in the right order rather than assembled by habit.
When a fold beside the mouth is a medical sign
Almost every fold beside the mouth is an ageing change, and the assessment is unhurried. A small number are the visible part of something else, and the difference is made by how the change arrived rather than by how deep the fold looks. A mouth corner that has dropped over hours or days is not an ageing change, and it needs medical assessment the same day; where it comes with weakness in an arm or a leg, difficulty speaking or swallowing, or a sudden severe headache, emergency care is the right route. Weakness on one side of the face, an eye or a mouth that will not close, drooling, or a change in speech all point at nerve function rather than at structure and need prompt assessment. Numbness in the lip, the chin or the cheek is a nerve finding rather than an appearance one. A lump that can be felt in the face or the jaw, and pain in the jaw joint with clicking or locking, are assessed as medical and dental problems rather than corrected with filler. A bite that no longer meets as it did, or a jaw that deviates as it closes, changes the whole question and is assessed before anything cosmetic is planned. Where none of this is present, the ordinary conversation about volume, descent and muscle can begin on a sound footing and at leisure.
What the examination looks for, and why
The fold has three possible producers and they are treated in different directions, so the assessment is designed to find which one dominates rather than to measure the fold. Almost all of it can be done in a few minutes in person, and none of it from a photograph.
The fold at rest and on movement
The face is looked at still, then speaking and smiling. A fold present at rest is being created by volume or by descent; a fold that deepens sharply in movement is being created by muscle pull. This single comparison is the most useful thing in the examination and it cannot be made from a photograph, because a photograph cannot show what the face does.
The jawline and the position of the tissue above the fold
Where the tissue above the fold has moved downward, the fold is a descent problem and lifting addresses it. Where it has not, the fold is about what lies beneath it. Looking at what sits above the fold rather than at the fold itself is what separates the two, and it changes the answer entirely.
Chin projection, seen from the side
The profile is examined, because a receding chin makes the fold beside it read as deeper than the tissue alone would explain. This is a structural cause and it is treated structurally. It is also the finding most often missed when the assessment is made from the front, which is how most people photograph themselves.
Skin quality and the direction of the lines
How thick and how elastic the skin is, and whether the fold runs as a single crease or as a set of finer lines. Skin quality is what determines how much of the appearance a topical can change, and the direction of the lines helps distinguish a fold created by descent from one created by the muscle.
The bite, the teeth and the jaw joint
How the teeth meet, whether the jaw deviates as it closes, and whether there is pain, clicking or locking in the joint. Where the bite is contributing, the assessment includes a dental opinion, because treating the soft tissue while leaving the bite produces a result that drifts back to the old pattern.
What has been done before, and when
Previous treatment in this area is recorded with the dates rather than in general terms, including how much was placed and where. The lower face accumulates material differently from other areas, and the total that has been used over the years changes what is sensible now, often more than the appearance does.
The ladder, from understanding the fold to surgery
The rungs run from working out what is folding to operations on the lower face, and the matching matters more than the order: each rung addresses one of the three mechanisms and is silent about the others. At the bottom, understanding the fold is a rung rather than a preliminary, because a fold that only appears in movement, a fold created by descent and a fold created by volume loss are answered by different things, and a treatment chosen before that is settled is a guess. Skin care and daily sun protection improve the surface and do not change the fold. Relaxing the muscle that pulls the mouth corner downward reduces the pull and softens the crease without adding anything, and it is a reasonable first move in a fold that is dominated by movement. Energy-based tightening produces a modest and temporary change and is no substitute for a lift. Filler along the fold and the chin is placed in small staged steps where volume is genuinely lost, because the lower face is where repeated volume accumulates most visibly over years, and because the tissue above the fold continues to descend meanwhile. Surgery that lifts the lower face addresses descent at its source; the fold usually improves and may not disappear. Doing nothing is a legitimate choice at every rung, and for a fold that only shows in a selfie it is the honest one. Nothing erases a fold that the mouth-corner muscle recreates every day.
Four ways this goes wrong
The first is to fill the fold repeatedly while the tissue above it keeps descending, because the volume is chasing a moving target and the lower face ends up heavier without the fold being resolved. The second is to treat the fold as a line to be erased, because it is recreated by movement every day and the volume required to erase it at rest is far more than the area can carry well. The third is to accept a plan that addresses the fold while saying nothing about the chin and the jawline, because the fold is read against the structures around it and a change to the fold alone can leave the lower face looking unbalanced. The fourth is to treat a mouth corner that has dropped suddenly, or one-sided weakness, numbness or a changed bite as an appearance problem, because those findings have causes that an aesthetic treatment does not address and that are easier to manage early. None of these are rare. Every one of them is easier to avoid at the start than to undo afterwards, and each begins with a treatment chosen before the mechanism was named.
If the decision moves toward treatment
This site deliberately stops short of describing operations, because the anatomy of the lower face and the surgery performed on it are explained properly elsewhere in this cluster. The Facial Aesthetic Surgery hub covers how a face is analysed and how balance is judged, and the assessment and technique are set out on Dr. Mao’s own site. Where the fold is part of a broader change in the lower face, the neighbouring concerns are Jowls and Weak Jawline, and where the chin is contributing, Recessed Chin and Chin Filler cover that ground. The comparison Facelift or Fillers sets the two directions of treatment side by side without picking a winner, and Mini Facelift and Deep Plane Facelift describe what the lifting operations do and do not reach. Where the intention is to soften a fold produced by movement rather than to add volume, wrinkle-relaxing injections covers the option, and the wider question of how much volume is being carried is addressed under dermal fillers and filler fatigue. Before any treatment, quality and surgical safety describes how safety is organised inside the hospital, and appointments explains how to arrange an assessment.
Marionette lines: volume, descent, muscle, or all three
Possible causes on the left and the ladder on the right, from least to most invasive. Note that the first rung is working out which mechanism dominates, and that each rung below answers one of them. The last line is the ceiling that no rung reaches.

Frequently asked questions
How do I tell whether my fold is caused by volume loss or by descent?
Look at the tissue above the fold rather than at the fold itself. Where the cheek and jaw tissue has moved downward, so that what used to be over the cheekbone is now beside the mouth, the fold is a descent problem and lifting addresses it. Where the tissue above is in its original position and the fold sits against a hollow, volume is the cause. Both are frequently present together.
Why is filler in this area treated differently from filler elsewhere?
Because the lower face is where repeated volume accumulates most visibly over years. The tissue here is mobile, the area moves constantly, and material placed in stages over a long period builds up in a way that becomes apparent rather than dramatic. That is why small amounts in staged steps are preferred to a single large correction, and why the total used over the years matters.
Will relaxing the muscle remove the fold?
It reduces the pull that deepens it and it softens the crease, particularly in a fold that is dominated by movement. It does not replace volume that has been lost and it does not lift tissue that has descended. Where those are also present, treating the muscle alone leaves most of the fold in place.
Does a facelift remove marionette lines?
It usually improves them and it may not remove them, and any plan that promises removal should be questioned. A lift addresses descent, which is one of the three mechanisms. The muscle continues to pull the mouth corner downward afterwards, and the fold that movement creates is still being created after the operation.
At what point should I stop treating this area?
When the fold has been softened and the lower face reads as rested rather than as filled. Continuing to add volume in pursuit of a fold that movement recreates is how the area becomes heavier without the fold being resolved. A plan should say what the endpoint is before the first treatment rather than after the fifth.
When should a fold beside the mouth be assessed urgently?
When a mouth corner has dropped over hours or days, when there is weakness on one side of the face, numbness, drooling, difficulty closing an eye or a mouth, or a change in speech or in the bite. Where any of that comes with weakness in a limb, difficulty swallowing or a sudden severe headache, emergency care is the right route.
