In short. The rungs in order, from understanding the fold through skin care, muscle-relaxing injectables, devices and filler to lifting surgery, with what each changes.
Key points
- Reducing muscle pull is the only rung that changes the fold without adding material or removing tissue.
- That makes it a useful first move, and a useful test: how much the fold softens tells you how much the muscle was producing.
The rungs run from working out what is folding to operations on the lower face. The matching matters more than the order, because each rung addresses one mechanism and is silent about the others.
Understanding the fold
This is a rung rather than a preliminary. A fold present at rest, one that appears only in movement, and one created by tissue arriving from above are answered by different treatments. Establishing which dominates takes minutes.
Skin care and sun protection
These improve the surface of the skin and slow the change in its quality. They do not change the fold, which is created by volume, position and movement rather than by the surface. Camouflage is an honest part of the plan, and more effective than most people expect.
Reducing the muscle pull
Where the fold is dominated by movement, relaxing the muscle that pulls the mouth corner downward softens the crease and adds nothing to the face. It does not replace lost volume and does not lift descended tissue. Where the fold deepens sharply when you speak, it is a reasonable first step.
A rung that adds nothing
Reducing muscle pull is the only rung that changes the fold without adding material or removing tissue. That makes it a useful first move, and a useful test: how much the fold softens tells you how much the muscle was producing.
Energy-based tightening
Produces a modest and temporary change in the quality and tightness of the surface. It does not lift descended tissue and is no substitute for a lift where a lift is the honest answer. Where it is offered as an alternative to lifting, that is worth questioning.
Filler along the fold and the chin
Placed in small staged steps where volume is genuinely lost. The lower face is where repeated volume accumulates most visibly over years, and the tissue above the fold continues to descend meanwhile, so the amount and the interval matter as much as the material.
Surgery that lifts the lower face
Addresses descent at its source, which is the mechanism no other rung reaches. The fold usually improves and may not disappear. The muscle continues to pull the mouth corner downward afterwards, and the fold that movement creates is still being created after the operation.
Where the ladder is misused
- Filling repeatedly a fold that is being created by tissue arriving from above.
- Buying a course of devices for a fold that needs a lift or volume.
- Treating the ladder as a sequence to complete rather than a set of matches to a mechanism.
What each rung does not fix is set out in what treatment does not fix. Why volume accumulates in this area is explained in why filler accumulates here, and the cases where treating is the wrong answer are covered in when not to treat.
Frequently asked questions
Why does the ladder start with understanding the fold rather than with a treatment?
Because the three mechanisms are answered differently, and a treatment chosen before the dominant one is known is a guess. Working out whether the fold is present at rest, appears on movement, or is produced by tissue arriving from above takes minutes and it decides which rung is relevant. It is the cheapest and most useful step on the list.
Should I try the muscle-relaxing injectable before filler?
Where the fold is dominated by movement, it is a reasonable first step, because it adds nothing to the face and it shows how much of the fold the muscle is responsible for. Where the fold is produced by volume loss or by descent, it does little. The assessment is what says which of those you are in.
How many treatments will I need?
There is no number that applies to everyone, and anyone who gives you one before examining you is guessing. What can be said is that small staged steps are preferred to a single large correction in this area, because the lower face accumulates material over years and because the tissue above the fold continues to change.
Risks & Limitations
- Repeated filler in the lower face accumulates over years, and an over-filled area is harder to correct than an under-filled one.
- Filler does not lift descended tissue, so adding volume while the tissue above continues to descend can leave the area looking heavier.
- A lift improves the fold and does not erase it, because the muscle recreates it with every expression.
- Bruising, swelling and unevenness are common after treatment in this mobile area, and the result is judged weeks later rather than immediately.
When to Seek Care
- A mouth corner that has dropped over hours or days, or weakness on one side of the face — seek urgent care.
- Numbness in the lip, chin or cheek, drooling, or a change in speech.
- A new lump in the lower face or jaw, or jaw pain with clicking or locking.
- A bite that no longer meets as it did, or difficulty closing an eye.
